A highly rewarding role which instills a strong sense of community, you will be a central contact within health units assisting in the transition care for Aboriginal and Torres Strait Islander people from Country SA who are embarking on patient journeys. With the advantages of working with a high level of autonomy and flexible working arrangements, you will coordinate the Patient Journey through the utilisation of key stakeholders involved in care coordination throughout the individual patient journey, including follow up care and supported transitions. A proven relationship builder, you will develop networks with Aboriginal Health Directorates to ensure increased access to culturally sensitive and appropriate services and information to support the patient journey, and work in partnership with other service providers to develop standard patient pathways and assist to translate this for individual patients. In addition, you will enhance, initiate and establish positive relationships with key stakeholders to ensure full understanding of the local services and cultural needs.
Essential to this role is considerable experience in working effectively with vulnerable Aboriginal people and non-Aboriginal people. Able to communicate to people of various cultures and backgrounds, you will have demonstrated experience in working with Aboriginal people, families and communities. Proven experience in the coordination of effective communication processes, negotiation and conflict resolution will be complemented by your ability to evaluate outcomes against set measures and identify and implement improvements.
